Instructions: Please use the SOP template below to write your laboratory-specific
SOPs. All sections on the SOP template must be completed. Please refer to the corresponding
Material Safety Data Sheet (MSDS)
from the same manufacturer of the chemical to add important information on the SOPs
per the manufacturer recommendations. All SOPs must be reviewed and signed by the
laboratory members and documented in the Laboratory Safety Manual binder.

Alternatively, you can customize the following chemical-specific SOPs to match your
laboratory chemical-specific operations. Do not cut and paste contents or
certain sections from the SOPs without careful review, as the SOPs in the library
must be customized for your specific laboratory operations.
Note: The SOP library is not a comprehensive list of the chemical inventory
of the UCLA campus, nor should it be implied that all of the chemicals included
in the inventory are currently in use on the UCLA campus.
When should a JSA be created? JSAs should be developed for all equipment
and specific operations that present safety hazards and/or concerns. Employees shall
be trained on and have a good understanding of the instructions and hazards listed
in the JSA prior to engaging in such work practices.
Who is responsible for creating JSAs? Although Supervisors and Safety Professionals
generally develop the JSAs, the forms shall be reviewed by workers performing the
job for feedback and suggestions. Often times, employees who routinely utilize equipment
and/or perform specific job tasks are better able to identify certain hazards associated
with the work, so it is critical that those individuals be involved in the JSA development
process.
The following Job Safety Analysis (JSA) templates are available for download in
Microsoft Word. These useful documents should be customized to specific equipment/operations
and used for employee safety training:
